The application timeline provides information on the general things needed to be accomplished a year prior to intended matriculation. You should complete these outlined tasks below in a timely fashion and do a thorough job of putting together a strong presentation of yourself. The AACOMAS application is only available online from the AACOM in mid-May of each application year. Submission of the AACOMAS begins in early June. The Texas Application Service is the centralized application required by University of Texas schools.

Complete and submit the HPE request form to the Health Professions Advisory Office once all of your required documents (copy of processed application, dean’s report, and all letters of recommendation) are on file. HPE Request Form (PDF)
The AACOMAS or TMDSAS transcript request form is submitted to Yale’s Office of the Registrar for them to send to the application services. The Health Professions Advisory Program does not require a transcript.
